By filling out and returning this form to us with as much detail as possible you will be assisting our research into the extent and possible ecosystem impacts of marine debris on northern Vancouver Island. Thanks for your help.

By submitting your photos or videos you grant Living Oceans Society (LOS) the right to publish the images for LOS purposes in any format including on the LOS website and in LOS publications or presentations, such as newsletters, brochures, fundraising appeals, media kits, public service announcements, grant proposals, reports, awards, thank-you cards, displays, mailings and invitations, etc. Copyright in your photo or video will remain with you. You can continue to use the photo or video in any way you want, including allowing others to use it.

In order to give LOS permission to use your photo or video, you confirm that (a) your photo or video is your own original work; (b) you own all the rights in the photo, including copyright; (c) you have the right to give LOS permission to use your photo or video for the purposes specified in these rules; (d) the photo or video is not defamatory and does not infringe any Canadian laws or violate the rights of any third party; (e) you have the consent of anyone who is identifiable in your photo or video, or the consent of their parent / guardian if they are minors; (f) you waive any moral rights that you may have in the photo or video; (g) you indemnify LOS against any claim, demand, action, suit, or other proceedings against LOS arising out of the use of the photo or video or any false or inaccurate statements.

If you do not want to grant LOS the permission set out in these rules, please do not submit your photo or video.
